Model Release, Assumption of Risk & Liability Waiver
Please read the three sections below, then complete the short form at the bottom of this page. It takes about a minute. You can grant or decline the model release — either choice is fine, and it has no effect on the photography services you have purchased.
I understand that photography sessions may take place on privately owned property or at outdoor locations with natural or uneven terrain, weather exposure, animals, fences, structures, and equipment. I voluntarily assume the ordinary and reasonably foreseeable risks of entering and using the location. I agree to respect the property, surroundings, animals, structures, and equipment, and to follow any instructions or restrictions given by the property owner or Vibrant Valley Photography.
To the fullest extent permitted by Colorado law, I release the property owner, Vibrant Valley Photography, and its owners, photographers, assistants, and affiliates from liability for injuries, accidents, illnesses, property damage, loss, or other incidents occurring on or related to the use of the property during the session, except for liability that cannot legally be waived. I agree to indemnify and hold them harmless from claims, damages, liabilities, costs, or expenses arising from the conduct of myself, my family members, guests, children, or pets. I am responsible for supervising any minor children and pets in my care, for ensuring that all participants exercise reasonable care, and for any damage they cause.
I grant Vibrant Valley Photography permission to photograph me and any minor child named in the form below for whom I am the parent or legal guardian, and to select, reproduce, publish, display, and use those photographs for its portfolio, website, social media, studio samples, printed promotional materials, advertising, exhibitions, competitions, publications, and other lawful business or promotional purposes. I understand these photographs may be edited, cropped, reproduced, published, or displayed without additional notification, approval, or compensation. This release does not authorize promotional use of any other adult participant’s likeness without that adult’s separate consent.
Declining is a real option. If you choose Decline in the form below, Sections 1 and 2 still apply, but your images will never appear in our portfolio, on our website, on social media, or in any advertising. Your session, your gallery, and your prints are completely unaffected.
This release supplements the Photography Services Agreement (v1.0).
By submitting the form below, I confirm that I am at least eighteen (18) years of age, that I have the legal authority to sign for myself and, where applicable, on behalf of the minor children named, and that I have read and understood this document. I understand that typing my full legal name below constitutes my electronic signature and has the same legal effect as a handwritten signature.
